His younger brother Tim Drew, a pitcher born in 1978, was also drafted in 1997 and was selected by the Cleveland Indians, the first time in history that two brothers were selected in the first round of one year. When the youngest of the family, Stephen Drew was chosen in the first round by Arizona in 2004, that was the first time three baseball players of the same family had been selected in the first round of the Draft.

J.D. made his major baseball debut shortly afterwards, playing his first game for the Cardinals on the 8th September 1998. He enjoyed a modest rookie season in 1999 with 39 RBIs and a batting average of .242 in 104 games. In 2000, he increased his offensive average to .295 and scored 57 points. He completed his season of 135 games played with 18 circuits and 17 stolen bases. In 2001, he averaged his best batting of .323 in a season, and his highest total of career circuits (27), and scored 73 points. Always patrolling the outside field of the Cardinals in 2002 and 2003, he finished the seasons with 18 and 15 circuits, and 58 and 42 produced points respectively. He played three times in the playoffs with Saint-Louis in 2000, 2001 and 2002.

At the end of 2003, the Cardinals exchanged Drew and Eli Marrero to the Atlanta Braves for the pitchers Adam Wainwright, Jason Marquis and Ray King . Although Drew played only one year for the Braves it was a very good season, with career-high hits (158), circuits (31) and points scored (118) in addition to finishing the year with 96 produced points. As a free agent, Drew then joined the Los Angeles Dodgers for the 2005 and 2006 seasons; he only played 72 games in his first season in California, but came back strongly with one of his best career years in 2006 when he set his personal high of 100 RBIs. He also hit 20 long balls and got 34 doubles, another personal record. Then J. D. Drew joined the Boston Red Sox of the American League, and in 2008 was selected for the MLB All-Star Game. Drew slammed respectively 24 and 22 circuits in 2009 and 2010, as well as scoring 68 points each time. He retired after the 2011 season.
